To create a new Survey:

1. Login
2. Click on My Surveys
3. Click on Create a New Survey
4. Follow the steps in the wizard

What are the different question types available?

Virtually any question type you can imagine. To view samples of all the different question types that are available click on:

• Login »  My Surveys »  Edit Survey »  Add Question

Can I customize all the scales/text etc?

Yes -- All aspects of a question are customizable.

Can I include Open-Ended Comments/ Other Option?

Yes. You can either add a separate question or include an "Other" option.
